VISA® DEBIT CARD PROGRAM
RULES, TERMS & CONDITIONS

  1. You must be a registered participant in the CAPITALCard program to be eligible to submit claims and receive awards.  Capital Media Group reserves the right to reject any registration.

  2. Account must remain in good credit standing in order to participate in the program.  Capital Media Group reserves the right to delay or deny payment of program rewards to delinquent credit accounts.

  3. There is no limit to the number of registrants per account.  One applicant per customer account will be named as the program administrator.  The administrator will become Capital Media Group’s point of contact regarding any questions or concerns regarding the CAPITALCard program.

  4. Upon registration, you must choose whether the customer account will participate in the CAPITALCard program on an individual basis or as a corporate entity.

    1. Participating on an individual basis would mean that the monthly claim is to be shared by one or more employees of the company and will be tracked by each individual’s SSN.  Capital Media Group bears no responsibility in determining how the monthly reward is divided amongst individuals within each reseller account.

    2. Participating as a corporate entity would mean that reseller account receives one card in the name of the reseller.  All rewards will be tracked by the company’s FEIN.

    3. Beginning March 1st, 2008 a customer account can sign up for the program as a corporate entity AND also have individuals registered for the program.

  5. All claims will be made on a monthly basis beginning from the 1st through the 10th of the following month.  Please contact your company’s program administrator with any issues related to the monthly claims process.  Capital Media Group will work with each company’s program administrator to resolve any issues.

  6. Capital Media Group reserves the right to make corrections or adjustments, if necessary, to cash balances on the card, if an audit determines this necessary.  All monthly claims will be totaled by account number and validated against that customer account’s purchases for the given month.  If the claims total exceeds the expected total then NONE of the claims for that account will be approved.  In this event, Capital Media Group will contact the customer’s program administrator in order to revise the individual claims.

    1. For example: RESELLERS, INC is eligible for $1000 in CAPITALCard rewards based on the company’s purchases in August, 2007.  The 12 sales reps of Resellers, Inc have each registered for the program and have each claimed their individual portions of the overall $1000 reward.  In error, the 12 claims submitted total $1001 which exceeds the reward available to Resellers, Inc.  In this event, NONE of the 12 claims submitted by Resellers, Inc will be approved.  Capital Media Group would contact the program administrator at Resellers, Inc. to have the pending claims corrected and re-submitted for approval.

  7. The products and program may change at any time.  Capital Media Group reserves the right to add eligible products, subtract eligible products, and raise or lower amount of cash awards during the eligible months.  Capital Media Group reserves the right to modify the terms of this offering at any time without advance notice.

  8. This offering is void where prohibited by federal, state or local laws.

  9. Capital Media Group reserves the right to terminate this program at any time.

  10. Cards may be expired 60 days after program termination, or on card expiration date, whichever is earlier.

  11. Card is valid anywhere Visa debit cards are accepted.  Capital Media Group will administrate the monthly claims process.  Once the funds are loaded onto the cards, Prepaid SolutionsUSA (a division of West Suburban Bank) will administrate the card transactions, usage, and administration.  For further details, please visit the website, or call the number, that appears on the back of the card.

  12. Capital Media Group determination of eligibility shall be final.

  13. You will not for any purpose be considered an employee or agent of Capital Media Group.  Participation in this program will not be construed as granting you any rights under any Capital Media Group employee benefit plan or otherwise.  You will never make any claim, or assist anyone else in making such a claim, to the contrary.

  14. Capital Media Group reserves the right to request additional documentation to process and/or pay any claims.  Failure to comply with requests for additional information and/or failure to provide requested documentation will result in termination from the program.

  15. Any falsification of claim form automatically eliminates individual from the program.

  16. CAPITALCard participants are responsible for all federal, state or local taxes and will receive an IRS 1099 form for any awards valued over $599 per annum.
